Check out Dale Carter’s post, Important, Little-known Resource for an Aging Parent, at her Transition Aging Parents blog. She writes about Medication Therapy Management (MTM), a service my husband and I wish we had known about for his mother, who died in January. It looks like an amazing service, one that I plan to learn more about for my two parents, both of whom take more than five medications each day.

  1. This is a service that more of the elderly and/or their caregivers need to use to prevent medication misadventures. The goal in using these services is to find pharmacists trained and certified in Medication Therapy Management (MTM). Sometimes physicians don’t have the time to conduct a thorough review especially when polypharmacy is occurring.
